Municipal Dementia Coordinator
The purpose of the service is to develop, organize, and deliver services to residents who are undergoing or have been diagnosed with dementia. Also, to increase the quality of life of citizens with dementia by supporting them in being self-reliant and an active part of the family and community for the longest time. Likewise, one of the tasks of the Municipal Dementia Coordinator is to act as a contact person and support a coherent civic process in an interdisciplinary and interdisciplinary geriatric collaboration, such as the Dementia Clinic, Municipal Medical Officers, Social Security and Home Services.
The service also includes relatives. To support relatives in relation to the changing role of the family, it is possible to receive guidance and instruction from the Municipal Dementia Coordinator.
